The only time I ever say anything is if I think the imminent injury is going to require any paperwork or clean-up on my part....in which case I have no problem snapping out a "Stop it!" at lightning speed toward the tiny beast. One time, the parents were oblivious as the older kid was literally choking the younger one...screw that. If they weren't worried about it, I wasn't either.
